About Create Fillable PDF Form
Create fillable PDF forms from the documents you already have, like a registration sheet, an order form or a checklist designed in Word and exported to PDF. Pick a field type and drag a box where the answer goes. You can add single-line text fields, larger text areas, checkboxes, option buttons, dropdowns and signature boxes. To add a heading or instructions to the page first, use Edit PDF.
Give each field a clear name, since that's what identifies the answer when forms are collected. Option buttons in the same group allow only one choice, dropdowns take one option per line, and any field can be marked Required. Font size 0 fits the text to the box. Fields that were already in the PDF are kept, and anyone with a standard PDF reader can type into the result.
How to use Create Fillable PDF Form

Choose the document that should become a form, or drop it on the page.
